Late Registration Form

WE DO NOT ACCEPT LATE SIGNUPS. However, if you missed signups you may submit your information below to be included on the WAITING LIST. Names are added to the list, and any openings are filled on a first come, first serve basis. Please do not contact us about the waiting list.

We will only contact you if we accept your application AND there is room to place you on a team. If you are placed on a team, a Parent or Guardian will need to sign the application, a late registration fee will apply. If you do not hear from us by June 1, 2009, no rosters spots were available.

PLEASE NOTE - WE WILL ONLY CONTACT YOU VIA THE EMAIL ADDRESS YOU PROVIDE WHEN SUBMITTING THIS APPLICATION. ANY INFORMATION WE NEED TO PROVIDE TO YOU WILL BE DONE USING THIS METHOD. WE WILL NOT BE CALLING ANY OF THE APPLICANTS, SO PLEASE CHECK YOUR EMAIL.
