Coach App Form

Interested individuals - fill out the following application form. Please note, that completing the application form is not a guarantee that you will be selected to coach. We will evaluate all applications and notify those selected no later than July 2009.

Before you can be selected as a head coach or assistant coach, you will be asked to sign a release form allowing FVF to perform a background check confirming you have not been convicted of any felonies or other selected criminal offenses.

NOTE: All game days will be Saturdays from 8:30AM to 1:30 PM
